<h2>Overview</h2> <p><strong>Intake Coordinator CASAC</strong></p> <p><strong>Remote work- after hours</strong></p> <p><strong>Healthcare staff can work anywhere….The BEST work with US!</strong></p> <p> </p> <p><strong>$35.71 per hour</strong></p> <p> </p> <p>A nationally recognized comprehensive Health and Human Services Agency, with over 60 programs across New York City and greater New York Area.</p> <p>Samaritan Daytop Village, serves over 33,000 New Yorkers annually within your neighborhoods and communities so our success depends on those we employ.</p> <p> </p> <p><strong>The Role</strong></p> <p> </p> <p>****This position must work after hours , weekends and possibly overnight after our outpatient clinics are closed.</p> <p> Under general supervision, is responsible for providing a variety of services in the initial Client-Agency meeting including coordinating the interview process and making the appropriate referrals inside and outside the Agency. </p> <h2>Responsibilities</h2> <p> </p> <p><strong>What You Will Do</strong></p> <ul> <li>Provides outreach presentations to stakeholders.</li> <li>Schedules and adheres to all interview appointments.  Conducts interviews and makes recommendations in compliance with Agency and funding sources rules and regulations.</li> <li>Prepares and gathers appropriate forms necessary for assessing individuals seeking treatment.</li> <li>Codes, edits and enters data as needed for billing and recordkeeping.</li> <li>Ensures accuracy for entered data and intake information.</li> <li>Consults with Clinical staff regarding recommendations for participation or referral.</li> <li>Assists in obtaining records for clients accepted for treatment.</li> <li>Disseminates copies of interview and other forms to appropriate departments and/or agencies.</li> <li>Offers education and information to clients and families regarding program philosophies and requirements for admission to program.</li> <li>Conducts community presentations as requested.</li> <li>Files and maintains client information in an appropriate manner.</li> <li>Prepares monthly statistics.</li> </ul> <h2>Qualifications</h2> <p><strong>Who You Will Be</strong></p> <ul> <li>High School Graduate or GED required.  Bachelor’s Degree preferred or Master’s degree in Social Work or related field required based on the program.</li> <li>CASAC-T Certification is required or QHP.</li> <li> 2+ years of related experience in counseling and case management</li> </ul> <p> </p> <p>#li-onsite</p>
